- Wet clothing, saturated linens, or unchanged adult diapers
- Poor hygiene or grooming, including disheveled appearance, feces under nail beds, missing dentures or eyeglasses, or wearing another resident’s clothing
- Call bells going unanswered due to inadequate staffing
- A loved one left sitting near the nurses’ station instead of participating in activities
- Trays of uneaten food left for residents who require assistance with eating
